It work for me and very well too. I think this can to be interest for many that want add more cool style to your contents in EZ.

Install:
Scompress the file in your extension directory, change permission to all files and enter in your back-end site. Then clear all cache and regenerate autoload array.
The extension will be run :)

Is there a way to split the toolbar width ?
For now, as the size is fixed it does not fit the size of the browser window.

I also regret that you didn't even ask the original patch's author about his point of view before submitting...
Indeed Tony (a colleague) already wondered about how to share it.
We've chosen to suggest it as a patch as it might be useful to users (as it is on some of our projects) and needed core changes that could eventually be integrated in eZ OE.
But dealing with a dedicated space doesn't make much sense for now, this may increase visibility (although this section is theorically deprecated) but I just think it has to be reviewed by a "oe guru" as Andre first, as there also might be other / better ways (or plans) to deal with that kind of needs.
That was the aim of the opened issue. If in the end he doesn't think it valuable or not a good idea to be included, it would then be possible to contribute it as an extra patch for interested users.
I think at least giving a patch instead of a full distribution may be more adapted in this case (and avoid confusion).
This said, your port to ez OE 5.0.4 is for sure welcome (did you need some changes for patch?), I would suggest you to add it on the related tracked issue.

I don't ask to participate to the project and I thought that have here all EZ publish Editor Online just ok for all was better.
why not try integrating the changes in the official version instead?

I think that in the svn of the official version there might be space for 'alternative' branches where new features are developed and tested. Did you ask to participate to the project leaders (AR)?
